Wang Hufeng, director of the Medical Reform Research Center of Renmin University of China, believes that this year is the first year of the "13th Five-Year Plan". It is necessary to continue to deepen the medical reform policy since 2009, but also focus on reform and innovation in the new era. Comprehensively deepening the reform of public hospitals and accelerating the promotion of hierarchical diagnosis and treatment are the key to deepening reform and innovation, and also laid the main tone of this year's medical reform.
The executive meeting of the State Council determined that the number of pilot cities for the comprehensive reform of urban public hospitals should be expanded from 100 to 200. In this regard, Wang Hufeng believes that after several years of exploration, the reform of public hospitals has formed a set of relatively mature reform paths and policy measures. This year, the direction of public hospital reform is clear, with the expansion of quantity and the further increase of pilot cities for comprehensive reform of urban public hospitals. At the same time, there are also qualitative requirements, and it is proposed to carry out comprehensive reform demonstration of county-level public hospitals. At the same time, from the perspective of supporting policies, it also puts forward supporting requirements for the reform of the management system, including personnel distribution, establishment, and salary. For example, it is required to improve the performance salary system of primary medical institutions, encourage pilot cities to formulate methods for determining the total performance salary of public hospitals, establish a distribution incentive mechanism related to job responsibilities and performance, and highlight the value of medical personnel's technical services.
Regarding the meeting's clear promotion of the hierarchical diagnosis and treatment pilot, Wang Hufeng believes that hierarchical diagnosis and treatment is an important part of my country's current medical reform. The solid and orderly promotion of the hierarchical diagnosis and treatment system indicates that my country's medical reform has entered a new stage and the medical service development model has begun to transform. In the past few years, we have focused on establishing and improving the medical security system, strengthening the patient cost protection mechanism, and simultaneously attaching importance to refined management services; from emphasizing system reforms including public hospital reforms to simultaneously attaching importance to the allocation and use of medical resources, hierarchical diagnosis and treatment will The supplier and the demander are organically connected, reform and development are unified, and management and services are combined.
He suggested that the promotion of graded diagnosis and treatment in China can neither copy certain foreign methods and implement a compulsory "one size fits all" approach, nor can it wait until all problems are solved before implementing it. It is more feasible to study and formulate local graded diagnosis and treatment diseases, one by one, and find a graded diagnosis and treatment method that is not only in line with the national conditions, but also acceptable to the people.
The executive meeting of the State Council proposed to comprehensively promote the centralized procurement of drugs in public hospitals, establish a traceability mechanism for drug ex-factory price information, and implement the "two-invoice system" from production to circulation and from circulation to medical institutions to make the intermediate link price increase transparent.
